The following documents are also necessary:
For requests made by a "legal representative"
・Documents proving their eligibility as a legal representative, such as their family register (limited to documents created within 30 days which do not include My Number; photocopies will not be accepted.)
For requests made by a "voluntary representative*"
・Power of attorney proving their eligibility as a voluntary representative (limited to documents created within 30 days which do not include My Number; photocopies will not be accepted.)
・Seal registration certificate of the party granted power of attorney (limited to documents created within 30 days which do not include My Number; photocopies will not be accepted) or a photocopy of documents only issued once to the authorized party, such as a driver’s license

* Disclosure requests made by a voluntary representative are limited to designated personal information (personal information including My Number).

(Notes)

  • Documents which can verify identity include driver's licenses, health insurance recipient cards, individual number cards (My Number cards), and residence cards.
  • For those making disclosure requests by mail, a copy of their residence certificate is required in addition to documents verifying identity (limited to documents created within 30 days which do not include My Number; photocopies will not be accepted.)
  • For those making disclosure requests by mail and submitting a photocopy of their individual number card (My Number card) as a form of identification, please submit a photocopy of the side on which the individual number is not displayed. When sending a photocopy of a health insurance card, make sure to omit (black out) the supplier number (記号) and personal number (番号). (Japanese is provided for reference as the health insurance card is in Japanese only.)
  • If you cannot submit a copy of your residence certificate for an unavoidable reason, please consult with the disclosure request counter beforehand.
  • Documents proving eligibility as a legal representative include family registers, excerpts of family registers, certificates of family courts, etc.
  • Documents proving eligibility as a voluntary representative include a power of attorney, documents that verify the identity of the voluntary representative, and a copy of the person's residence certificate (limited to documents created within 30 days which do not include My Number; photocopies will not be accepted). Please also submit the seal registration certificate of the party granted power of attorney (limited to documents created within 30 days which do not include My Number; photocopies will not be accepted). Please also attach a copy of documents only issued once to the authorized party, such as a driver’s license.
  • If the name and/or address written on the documents are different due to marriage or moving, please prepare other documents with the name and address listed on the request form.
