Disembarkation card for foreign nationals (translated version)

There are three types of disembarkation cards for foreign nationals submitted at immigration when entering Japan: (1) cards with Japanese, English, and Traditional Chinese; (2) cards with Japanese, English, and Simplified Chinese; and (3) cards with Japanese, English, and Korean.

The Immigration Services Agency has created translated versions in a total of nine languages, including these five languages, to explain the items on the card. Please refer to these translations when filling out the actual disembarkation card for foreign nationals.

When filling out the disembarkation card for foreign nationals, please write in either English or Japanese.
