    (Note) The following notification format is for reference only. You do not necessarily need to follow this format. However, please try to use this format when possible to avoid incomplete notifications.

  • Documentation to prove the contents of the notification (if necessary)
  • Presentation of proof of identity (or enclose a copy of your identification if you mail it in)

    (Note) Depending on the content of the notification, additional proof may be required.

  1. Notification items

    • (1) Payment status of compensation to the specified skilled worker compared to other employees to determine compensation owed to the specified skilled worker
    • (2) The number of affiliated employees, the number of new employees engaged in the same work as the specified skilled worker, the number of retired employees, the number of missing employees, and a breakdown of those numbers into the numbers of Japanese people and foreign nationals
    • (3) The status of health insurance, pension insurance, and employment insurance coverage, and the status of procedures for industrial accident compensation insurance coverage
    • (4) The status of the safety and health of the specified skilled worker
    • (5) The amount and breakdown of the cost of accepting the specified skilled worker

  • Attach materials (such as a payroll book) showing the payment status of wages.
  • In the case of non-voluntary termination, attach a copy of the worker roster.
  • If the whereabouts of the specified skilled worker are unknown, a separate notification of employment difficulty is required.

(Note) To avoid reducing the size of the copy, uncheck "Reduce page to fit paper size (K)" in the print dialog box before printing.

Where to submit Online:Notifications may be submitted online, through the Immigration Services Agency's e-Notification System. (Click here to be taken to the e-Notification page.) Advance user registration is required.
In person:The Regional Immigration Services Bureau (excluding airport offices) with jurisdiction over the head office address of the affiliated organization of the specified skilled worker (Contact the Regional Immigration Services Bureau or Immigration Information Center. Tel: 0570-013904.)
Notification by standard mail: Enclose a copy of a document certifying your identity and forward to the Regional Immigration Services Bureau with jurisdiction over the head office address of the affiliated organization of the specified skilled worker. Please write "特定技能届出書在中" in Japanese in red on the front of the envelope.
Counter hours for in-person submission Weekdays from 9:00 a.m. to 12:00 p.m., and 1:00 p.m. to 4:00 p.m. (The day or time may vary depending on the procedure, so contact either the Regional Immigration Services Bureau or the Immigration Information Center. Tel: 0570-013904.)
