    (Note) The following notification format is for reference only, and it is not absolutely necessary to follow this format. However, please use this format where possible to avoid incomplete notifications.

  • Presentation of proof of identity (or enclose a copy of your identification if you mail it in)

    (Note) Depending on the content of the notification, additional proof may be required.

  1. Notification items

    • (1) The name, date of birth, sex, nationality/region, place of residence, and residence card number of the specified skilled foreigner the notification concerns
    • (2) The period of time when the unfair act or misconduct in relation to laws and regulations related to immigration or labor took place, when said unfair act or misconduct became known, and the response to said unfair act or misconduct
    • Details of the unfair act or misconduct, in relation to laws and regulations related to immigration or labor

    * If more than one instance of misconduct has taken place or more than one person was involved, additional sheets of paper may be used.

Where to submit Online:Notifications may be submitted online, through the Immigration Services Agency's e-Notification System. (Click here to be taken to the e-Notification page.) Advance user registration is required.
In person:The Regional Immigration Services Bureau with jurisdiction over the head office address of the affiliated organization of the specified skilled workers (excluding airport offices). (Contact the Regional Immigration Services Bureau or Immigration Information Center. (Tel: 0570-013904.)
Notification by standard mail: Enclose a copy of a document certifying your identity and forward all items to the Regional Immigration Services Bureau with jurisdiction over the head office address of the affiliated organization of the specified skilled workers. Please write "特定技能届出書在中" in Japanese in red on the front of the envelope.
Counter hours for in-person submission Weekdays from 9:00 a.m. to 12:00 p.m., and 1:00 p.m. to 4:00 p.m. (The day or time may vary depending on the procedure, so contact either the Regional Immigration Services Bureau or the Immigration Information Center. Tel: 0570-013904.)
Notifications may also be submitted by mail.
