Individuals subject to procedure Mid to long-term residents who possess a status of residence of "Dependent" (limited to those conducting daily activities as a spouse), "Spouse or Child of a Japanese National" (limited to those with the status of spouse of a Japanese national), or "Spouse or Child of a Permanent Resident" (limited to those with a status of spouse of a permanent resident) who have gotten divorced or whose spouse has died  
Notification period Within 14 days from the date of the change
Notifier The mid to long-term resident
Notification items In addition to the name, date of birth, sex, nationality/region, address, and residence card number of the mid to long-term resident, please include the notification items below as required.
  1. Notification of divorce from spouse
    Date of divorce
  2. Notification of death of spouse
    Date of death
Where to submit Notifications can be submitted using the following three methods.
  1. Internet:
    The Immigration Services Agency's e-Notification System is available 24/7, 365 days a year. Notifications can be submitted online with this system, getting rid of the need to visit a Regional Immigration Services Bureau in person.
    First-time users must register to use the system (this links to the e-Notification System registration information page)
    * The submission of documentation certifying the notification items is not required.
  2. In person:
    Please submit the notification form along with your residence card to your nearest Regional Immigration Services Bureau.
    The day or time may vary depending on the procedure, so please contact either the Regional Immigration Services Bureau or Immigration Information Center (Tel: 0570-013904).
    * The submission of documentation certifying the notification items is not required.
    * For notification items, any format is acceptable. Reference forms for each notification reason are provided below for convenience.
  3. Mail:
    Please include a copy of your residence card with the notification and write "届出書在中" in Japanese in red on the front of the envelope. The envelope should be sent to the following address:
