Individuals subject to procedure Organizations that have started or finished accepting mid to long-term residents with a work-related status of residence (excluding "Artist," "Religious Activities," "Journalist," "Technical Intern Training," and "Specified Skilled Worker") or the "Trainee" status of residence (excluding organizations with an obligation to make notification of the status of foreign national employment in accordance with the Employment Measures Act), and organizations that have started or finished accepting mid to long-term residents with the status of residence of "Student"  
Notification period Within 14 days from the start or end of the acceptance of the mid to long-term resident
Notifier Staff of the affiliated organization

    (Note) Any format is acceptable for notifications. Sample forms are provided below for convenience.
  • Presentation of documents or other materials proving identity
    (Note) The submission of documentation certifying the notification matters is not required.
Notification items
  • Matters written in the notification form
    Name, date of birth, sex, nationality/region, address, and residence card number (including the registration number of the certificate of alien registration, which is considered to be the residence card) of the mid to long-term resident
    In addition to the general items described above, include the notification items below as required.
  1. If the acceptance of a mid to long-term resident with a work-related status of residence (excluding "Artist," "Religious Activities," "Journalist," "Technical Intern Training," and "Specified Skilled Worker") or the "Trainee" status of residence has begun:
    1. Date of acceptance of the mid to long-term resident
    2. Details of the activities performed by the mid to long-term resident

    2. Graduation, withdrawal from school, expulsion, and/or other reasons for ending the acceptance of the mid to long-term resident

    (Note 1) Can be printed on standard Japanese A4 paper.
    (Note 2) To avoid reducing the size of the copy, uncheck "Reduce page to fit paper size (K)" in the print dialog box before printing.
Where to submit Internet: You can submit the notification via the internet by using the Immigration Services Agency's e-Notification System. Advance user registration is required.
Counter: The Regional Immigration Services Bureau with jurisdiction over your place of residence (Please contact a Regional Immigration Services Bureau or an Immigration Information Center (Tel: 0570-013904).)
Mail: Please send the notification to the Registration Department, Tokyo Regional Immigration Services Bureau. Please write "届出書在中" in Japanese in red on the front of the envelope.
